Before the start of each school year, all athletic participation forms required for the Bay Athletic Department (Bay High School and Bay Middle School) must be completed online through FinalForms at http://bay-oh.finalforms.com/.
Once you create a FinalForms account, you can also upload the completed forms by the physician by scanning the documents to your computer and then uploading them to FinalForms using the small folder icon in the upper right-hand corner of the page.
